Reports of loss and damage claim (Form S-9A), ca. 1980-[ongoing].

The ABC Board operates a chain of stores across the state to sell alcoholic beverages both at retail and wholesale. This report (Form S-9A) is to be prepared if an error (overage or shortage) is discovered after the delivery truck has departed. Merchandise broken in shipment or when delivered is to be written on this form also. Information provided on the the form includes the date, invoice number, store number, carrier name, driver name, truck number, vendor name, date discrepancy or damage to merchandise discovered, and detailed facts about the discrepancy or damage. The form is then signed by the store manager and by a store employee who has witnessed the inventory. The original and duplicate copies are submitted to the Division's Central Office and the information is keyed into the computer, resulting in the creation of the Register of Store Differences (series S-54-30). The third copy is to be retained at the store.
